In the 3000s B.C., the noses of sex workers in India were cut off to mark them in society. Such cases were treated by a physician called Sushruta Samita, who was known as a healer at that time, using tissue from the forehead. In 1887, after Roe made the definition of closed rhinoplasty to the scientific community, people learned that they could change their noses not only for breathing but also aesthetically. The first rhinoplasty was a closed rhinoplasty. Pay attention to this point… Closed rhinoplasty was a difficult surgical procedure to learn because no one else could see it. Therefore, out of necessity, fifty years after the discovery of closed rhinoplasty, open rhinoplasty was discovered, or rather defined. What is an Arched Nose?

The word arch is a definition that starts with Seljuk architecture and continues with the Ottoman Empire. At least that’s what I know. If you have a protrusion on the back of your nose that distorts the linearity upwards, it means you have an arched nose. In our country, as you go to the Black Sea, the rate of arched noses increases. This is not a situation that challenges surgery. In fact, the extent of change is enjoyable as it portrays us as noble people…                         

What is Septum Deviation?

There are two walls on the right and left sides of our nose. In the middle is a wall that divides our nose into two. This wall that divides our nose in two is called the septum. It has a similar meaning in Latin. The structure we call septum consists of cartilage on the lower side of our nose and a fine bone on the upper side. This structure, which we call the septum, starts from the nostrils and continues until the opening we call the pharynx. Sometimes our nose is crooked congenitally and sometimes from received blows. Sometimes the septum curves even though the nose is in place. When the septum is crooked the condition is referred to as septum deviation. Depending on the degree of this condition, it can cause respiratory distress. Some septum deviations can be corrected without touching the roof of the nose, but some may require changing the shape of the nose. The path to be followed can be determined with a simple examination.

What is a Thick-Skinned Nose

Now we will do a simple study with you. Check the thickness of your skin at the root of your nose. This is almost the thinnest part of the nose skin. If you go down slowly, your nose skin starts to thicken approximately at the midpoint of the nose. This point where the change occurs is very clear. This is a normal anatomy. On the other hand a thick-skinned nose is not an absolute formula or criterion for this thickening, but is completely shaped by experience. Again, I can give you some hints so that you have an idea;

  • Thick-skinned noses can have an orange peel-like skin (these are rhinophyma of various stages)
  • It is more difficult to feel cartilage at the tip of the nose in thick-skinned noses.
  • In thick-skinned noses, the lower half of the nose appears unrefined.
  • Thick-skinned noses are more oily

Of course, this does not mean that our thick-skinned patients cannot have rhinoplasty. It just means that there will be more swelling during the healing process and it will be more difficult to overcome and the time required for the nose to settle fully will be a little longer, that’s all.

What is a Crooked Nose

In fact, we all have asymmetry in our faces. Because during the embryonic development of our face, our right and left sides develop from different sides of the growth plate and these structures meet at the middle point. Since our nose rises on this pedestal, there are differences on the right and left sides.

It is not as important a problem as some people tend to exaggerate, on the contrary, it is our natural structure. Here is this asymmetry, which we say is natural in the skeleton of our middle face, where our nose is based, if it is too much, our nose can look very crooked. Of course, some traumas can also cause nasal curvature. The shape of the curvature in the nose affects the success of the nasal curvature correction surgery. We talked about heating and humidifying inhaled air within the functions of our nose. Our nose has a shelf system that performs this task in an ergomomic way as the air passes, which is called the concha. These shapes resemble horns extending from the side wall of the nose. These structures are commonly known as nose meat. Conchas can grow excessively, sometimes structurally and sometimes triggered by allergens. There is no criterion to describe excessive, however if the concha has reached the size to prevent the passage of air entering the nose, concha hypertrophy, that is, concha growth has occurred. If there is an enlargement in the turbinate, that is, in the concha, that will cause breathing difficulties, it should be reduced by surgical intervention and the airway should be opened. Surgical procedure for turbinate enlargement can of course be performed together with rhinoplasty.


Can Snoring Be Corrected With Nose Aesthetics?


As the air enters through the nostrils and flows towards our nasal passages, it should proceed along a regular route, that is, with a laminar flow, and should only flip at the high points of the nose. This described flow is a normal anatomical flow. If this flow is disrupted, a turbulence that would not have happened before can occur and cause snoring. This flow can sometimes be caused by concha growth, sometimes by deviation of the septum, and sometimes by nasal deformity. If the cause of snoring is due to airflow deterioration and the airflow is impaired as a result of these anatomical structures, your snoring problem will be resolved during rhinoplasty.

However, I want you to know that this is not the cause of most snoring. The most common cause of snoring is a loose and sagging soft palate. If this is the case the problem should be corrected with surgical procedures on the soft palate or uvula.

The tip of the nose, that is, the tip area, is one of the sub-units of nasal aesthetics. If you really don’t have any other problems with your nose, you can have nasal tip aesthetics only. Let’s explain this a little. Non-surgical rhinoplasty, What is nasal filling?


Nasal filling, also known as non-surgical rhinoplasty, is one of the popular applications of recent years. The earliest age limit for rhinoplasty is seventeen for girls and eighteen for boys. This one-year age difference is due to the difference in ossification between boys and girls. However, rhinoplasty can be planned at an earlier age in severe respiratory distress of the type closely related to the shape of the nose, and in some congenital deformities (such as cleft lip nose-CLN).

What is Open Rhinoplasty

It is a method that was defined fifty years after the discovery of closed rhinoplasty. In principle, an incision is made on the middle wall, which we call columella, at the base of the nose, and the nose skin is lifted from the underlying cartilage and bone and the nose is shaped. Surgical scars on the columella are also not uncommon. In fact, open rhinoplasty emerged because surgeons who practiced closed rhinoplasty had been practicing this technique without teaching and spreading it like a cult for many years. However, nowadays, the method is defended with a commercial discourse such as “the tip of the nose is opened to see it better”. However, the truth of the matter is that every method gives good results in the hands of its master. And those who say that the surgeon cannot interfere with the tip of your nose in closed rhinoplasty make me laugh as much as the old surgeons who say that the gallbladder cannot be removed properly with laparoscopic surgery and must be done open.


What is Closed Rhinoplasty

Closed rhinoplasty is a surgical technique performed by making an incision in the columella without collecting the skin at the root of the nose. This is why it is commonly known as scarless rhinoplasty. Of course, there are incisions in closed rhinoplasty, otherwise we would not be able to reach the cartilage and bone. Only these incisions remain inside the nostrils. I think the most advantageous aspect of closed rhinoplasty (traceless closed rhinoplasty) is that the result is visible throughout the entire procedure. In closed rhinoplasty, the elasticity of the nasal skin does not change throughout the case and you continue your procedure by seeing the result of the nose in front of your eyes at any moment. Of course, the absence of an incision and suture marks in the columella also has a serious perceptional advantage. In my adventure with plastic surgery that I started in 2002, I was instructed in closed rhinoplasty, I had extensive experience in open rhinoplasty between 2008-2011 and I continue to focus on closed rhinoplasty again. What is Ultrasonic Piezo Nose Aesthetics

It is a small saw shape developed to cut the bone on the side wall of the nose. The difference is that what moves the saw is ultrasonic sound waves that vibrate tens of thousands of times per second, causing the bone to be cut like butter. The tip of the piezo saw is equipped with a continuous water washing system to avoid burning the tissues. Using this device to cut bone is just a personal preference, it makes no special contribution to the rhinoplasty technique.

What is Micromotor Nose Aesthetics

The micromotor saw option was available before the invention of the ultrasonic piezo nose saw. The basic principle and application area are the same. The only difference is that instead of vibrations, bone tissues are cut with miniscule back and forth swings. Again, in my opinion, using this device to cut bone is just a personal preference, it makes no special contribution to the rhinoplasty technique. The device can give good results in skillful hands.

What is Nose Aesthetics with Laser

I can understand rhinoplasty with a micromotor and ultrasonic piezo saw. After all, everyone cuts the tissue with whatever comes easy for them. The principle of both is mechanical. However, the laser issue is very different. As a people, we think that whatever is a product of the Japanese cartoons of our childhood, even if it is an American invention, is the most beautiful and the most wonderful thing ever. In fact, that school of thought is pure nonsense. The idea that in rhinoplasty, cutting should be done with a laser was a commercial fantasy that tried to gain hold. However, the color and water ratios of the tissues in the nose created more tissue trauma than imagined because there is not enough tissue selectivity.   • Like I said, we do not use tampons, only a silicone splint will be in the nose so that there is no adhesion to the fresh incisions and the swelling does not block your airway.
  • I will discharge you the next morning after checking you over in your room and giving your prescription.
  • On your first control we will remove the plastic plaster called a splint approximately 7 days later to be replaced with a paper bandage which will stay for 7 days.
  • On the next meeting 7 days later, you will be at our clinic to have the paper tapes removed. If it is inconvenient for you to come to the clinic, it is possible to remove the tapes by soaking them in the bathroom.
  • We like see patients close to us at the end of the first month, on the 3rd, 6th, 12th The frequency of this plan can be increased depending on your status. We can usually see our overseas patients through photographs after removing their splint and only see them one year later.

Will my nose look natural?

Is the post operative period after rhinoplasty very painful?

  • Curvatures of the nasal skeleton
  • Valve stenosis at the nasal entrance
  • Paralyzed (paralysis) Nose (with the muscles of the back of the nose not functioning)
  • Empty Nose Syndrome
  • Body Dysmorphic Disorders

Except for the last three issues in the list of respiratory problems above, all other causes can be corrected during rhinoplasty and your breathing can be facilitated simultaneously. The last three issues will require additional treatments.

Will my nostrils become smaller?

Yes, if necessary. In my own practice, I reduce the nostrils of 95% of my patients during rhinoplasty. I think it is a shortcoming not to downscale the nostrils of a nose that gets downscaled in every other dimension. This process can also be done by minimizing the fleshy structure of the nostrils with or without reducing the actual holes. Confused? For your information, the fleshy edges of the nostrils can be finetuned without downsizing the nostrils. It is important to reduce the nostrils of some patients without causing respiratory problems.

Is  Male Nose Aesthetics Different?

There are some differences between male rhinoplasty and female rhinoplasty in terms of both the surgical process and aesthetic approaches. These are as follows;

  • The male nose is generally thicker, so the swelling is slightly more and takes a longer time to settle down.
  • Since the male bone structure is thicker, more care should be taken in terms of bruising and swelling.
  • The skin of the male nose is more oily, and it is more difficult to clean and care for the skin after surgery. If black spots, also called comedones, are allowed to accumulate, the nostril reduction incision can become somewhat obvious.
  • The male nose should not be given a curve, otherwise it will take on a feminine look, which I think is a disaster.
  • The upturn of the male nose (nasolabial angle) should not be more than 90 degrees. While a nose tip uplift of 95-100 degrees in women looks nice and attractive, it is not attractive in men


When is the patient discharged?

Since rhinoplasty surgery is done under general anesthesia, our patients spend the first night in the hospital and are discharged the next morning. Of course, in some special cases, we discharge our patients on the day of the procedure.


Things to Know after Nose Aesthetics


  • Rhinoplasty has a special prescription. We will give you your prescription when you leave the hospital. Do not use anything other than what is on the prescription without informing us.
  • Remember that you had surgery, you might see some blood, this is normal.
  • We will tell you how to do special cleaning and washing applications in addition to the basic treatment drugs in the prescription.
  • Never, ever, ever hit your nose. This is the most important issue. Whatever the size of the blow you receive, talk to your doctor so we can be alert to various possibilities. I emphasize this especially because it is the most frequently told lie…
  • Do not turn left or right while sleeping, use a sleeping pillow.
  • Come to your check-ups regularly.
  • Do not get into unnecessary stress now that your surgery is over. Stress can raise your blood pressure and cause bleeding and unnecessary swelling and bruising.
  • Do not try to draw unnecessary and meaningless conclusions about your nose result when you have just had the operation and the nasal swelling is still evident. It will take a year for the nose to fully settle after the operation.
  • Be positive and enjoy. Experience reveals that positive people heal better.

What is the complication rate in rhinoplasty?

Certainly. There are many standard surgical complications in rhinoplasty, such as crookedness, excessive bleeding, adhesions, graft shifts, excessive swelling and bruising, perforation of the septum, infection, suture reaction, reactions to drugs used, anesthesia complications… The average complication rate in international publications is around 10%. This varies from surgeon to surgeon. My average is around 6%. Half of them are simple issues that can be remedied in 5-10 minutes chatting in the office environment. My rate of re-entering the operating room is 2-3%. There is a saying in surgery that you should not forget: “The doctor who doesn’t do surgery has no complications”.

Theoretically, it takes a year for a wound to heal. Of course, progress is being made gradually at each step of this process. The settlement time of secondary cases can take longer than one year.

Can rhinoplasty be reverted?

There is a partial chance of reverting rhinoplasty. By taking cartilage and fascia tissues from other parts of the body, we can give your nose a new arch and increase its volume. Has the patient had a bad nose surgery and is looking for his former nose, or does he miss his former rough arched state, which was more characteristic, although there was no problem with his surgery? We must describe the problem accurately. First of all, I need to do a detailed examination to be able to explain what and to what extent I can fix it. It should be kept in mind that when rebuilding a previously operated nose, we might need to use some materials as grafts from rib cartilage, ear cartilage, and a membrane in the temple area.
